If your request for funding is approved, you will receive copies of the approved Logic Model and Budget along with your contract, and again, electronically, shortly thereafter. It is imperative that you keep these approved documents, which will become the basis for your reporting.

PLEASE NOTE that the Interim and Final Report Forms have recently been revised. Be sure to use the versions accessed by the links below.
Interim and Final Reports must be submitted online in accordance with the outlines below. They must include updated versions of the approved Logic Model and Budget (see above) and a narrative that provides detail beyond your Logic Model about your accomplishments and challenges to date. In the case of the Interim Report, the narrative must include an explanation of how you envision moving forward for the remainder of the grant year.
If you would like to request a modification for the program budget, please use the Budget Modification Form and submit it to your Program Officer. ALL budget modifications MUST be approved by the Foundation.
