Understanding Our Application Types

The Healthcare Foundation of New Jersey currently utilizes five different application forms – each with their own processes, requirements, and deadlines. Click below to learn more about each application type, and what is needed to apply.

The Word documents provided are for reference only, and should not be submitted with your application.

Standard Grants
  • Application Deadline: Quarterly deadline.
  • Materials Needed: Text of Standard Application, Grant Application Logic Model,Grant Application Budget
  • Supplemental Documents Needed: Logic model, project budget, organization budget, audited financials, management letter, board member/key staff lists, IRS form 990, IRS 501c3 certification letter.
  • Details: HFNJ’s most common application type is a 12-month grant to support a specific project. Every proposal must adhere to HFNJ’s Grant Guidelines and be submitted through the Foundation’s online application portal.
Renewal Grants
Special One-time Grants

Details: Each year the Foundation considers a limited number of proposals for funding of less than $35,000.  Such proposals are considered on a rolling basis.  Every proposal must adhere to HFNJ’s Grant Guidelines and be submitted through the Foundation’s online application portal. Grants considered in this limited, fast-track pool are generally awarded as one-time support and not considered for renewal. They should address urgent circumstances and/or be time sensitive, or fund limited-time initiatives. Proposals that request HFNJ support for projects larger in scope may be declined or referred to the regular granting process.

Capital Grants
  • Application Deadline: Quarterly deadline.
  • Supplemental Documents Needed: Project budget, organization budget, audited financials, management letter, board member/key staff lists, IRS form 990, IRS 501c3 certification letter.
  • Text of Capital Application, Grant Application Logic Model,Grant Application Budget
  • Details: HFNJ has long supported organizations making capital requests to fund the construction or renovation of clinical spaces; the purchase of equipment such as MRI machines or surgical equipment; and other such capital requests. Because these projects often do no fit into the one-year timeframe of our regular grants – with the benefits to the community in particular spread out across many years if not decades – we have recently instituted a new application form just for capital requests.
Multi-year Grants
  • Application Deadline: Applications only accepted by invitation
  • Supplemental Documents Needed: Logic model, project budget, organization budget, audited financials, management letter, board member/key staff lists, IRS form 990, IRS 501c3 certification letter.
  • Details: The Foundation will accept a limited number of grant requests to fund projects that take place over multiple years (up to three). Grants can be in any program area and must outline why a multi-year request will be helpful for project implementation, and should demonstrated realistic, clear goals with articulated short- and longer-term goals that can be accomplished with a 2-3 year timetable. To be eligible to submit a multi-year request, organizations must have a history of successful grant projects with The Healthcare Foundation of New Jersey and must have prior approval from the Executive Director/CEO. Organizations that would like to be considered for a multi-year grant should contact our office at (973) 921-1210 to arrange a time to speak with our Executive Director/CEO about this opportunity.
